Private Health Insurance Claims
Each health fund has their own requirements and processes however we
find most require the following to be submitted for private health
claims:
- Your invoice (proof of payment) provided to you by CPAP Specialists once payment is made
- Your sleep study report (Proof of diagnosis)
- The provider number of your diagnosing doctor/physician.
Depending on your level of cover and who you are insured with you may receive a rebate.
